ENROLLMENT PROCEDURES
1st Semester Academic Year 2017 - 2018
I. NEW Students/Transferees
STEP 1 . Submit the result of Entrance Examination to the Dean/
Program Head/ Enrolling Officer and secure plotting form.
Note: Submit proof of family income such as ANYof the following for prioritization of tuition fee subsidy to
“financially disadvantaged but academically able students” (2017 General Appropriations Act, President
Duterte’s special provisions for CONDITIONAL IMPLEMENTATION of “No Tuition Fee” policy).
- Income Tax Return (ITR)
- BIR Form 2316
- BIR Certificate of Exemption from Filing of ITR
- Barangay Certificate of Indigency
- Certification from the DSWD
- Overseas Filipino Worker (OFW) Certificate
- Employment Contract for OFWs
- Other government-issued documents that indicate the income earnings of member(s) of the household who would be responsible for the financing of the student’s cost of education
STEP 2. Proceed to the Gymnasium for ROTC or CWTS enrollment.
STEP 3. Copy the schedule code of the subjects to be enrolled from the Bulletin Board at the gymnasium and submit to the Enrolling Officer for approval.
STEP 4. Present approved plotting form to the encoder (GYM)
STEP 5. Proceed to the Cashier's Office for assessment and payment of fees.
Tuition Fee will still be included in the assessment for
billing reference to CHED/ DBM/ LGU & other private
scholarship or tuition fee subsidy grantors if qualified.
STEP 6. Proceed to the Medical/Dental Office for medical check-up and certification
STEP 7. a. Proceed to the Office of the Student Affairs and secure a
copy of the Student Handbook.
b. Apply for Identification Card (ID).
II. Old Students
STEP 1 . Submit clearance and updated evaluation form to the Dean/
Program Head/ Enrolling Officer for the issuance of plotting form.
Note: for 3rd year and below, including 4th year who are not
graduating this AY 2017-2018, submit proof of family
income such as ANYof the following for
prioritization of tuition fee subsidy to “financially
disadvantaged but academically able students” (2017

C. OLD STUDENTS:
